  • Completed and Signed Application for Enrolment
  • Signed Enrolment Contract
  • Signed Parent / School Partnership Agreement
  • Photocopy of Record of Immunizations
  • Photocopy of Birth Certificate
  • Two Recent Photographs

The City of Ottawa Public Health Department requires that parents or guardians of children attending a school in Ottawa provide the school with a complete record of their child's immunization.

School Entry Immunization Forms will be provided by the Admissions team during registration.
